Author’s perspective: I wanted to write this post because the neck and shoulder are such close neighbours that pain often gets blamed on the wrong one. Someone with a pinched nerve in the neck may spend weeks stretching their shoulder. Someone with a frozen shoulder may buy a cervical collar. Knowing where the pain actually comes from saves time, money and frustration. I have kept this post focused on telling the two apart, and on safe next steps.
Why the confusion happens
The nerves that supply the shoulder, arm and hand all start in the neck. If a nerve root in the neck is irritated, pain can travel down to the shoulder, upper arm or even the fingers. This is called referred or radiating pain. So a problem in the neck can feel like a shoulder problem.
At the same time, a stiff, painful shoulder causes people to hold their neck and upper back tensely, which can create neck pain too. The two can overlap.
What cervical problems look like
“Cervical” refers to the neck part of the spine. Common neck problems include:
- Cervical spondylosis: age-related wear of the discs and joints in the neck.
- Cervical radiculopathy: irritation or compression of a nerve root, often from a disc bulge or bony change.
- Muscle strain: from poor posture, long phone use or sleeping awkwardly.
Typical signs that the pain is coming from the neck:
- Neck pain and stiffness, especially turning or tilting the head
- Pain that travels from the neck over the shoulder blade and down the arm
- Tingling, numbness or “pins and needles” in the arm or fingers
- Weakness in grip or certain arm movements
- Pain that changes when you move your neck, rather than your arm
- Headaches starting at the back of the head
What frozen shoulder looks like
Frozen shoulder (adhesive capsulitis) is a problem in the shoulder joint itself, where the capsule becomes tight.
Typical signs that the pain is coming from the shoulder:
- A deep ache in the shoulder, sometimes spreading to the upper arm, but usually not below the elbow
- Stiffness in all directions, especially rotating the arm outward
- Difficulty reaching up, behind the back or across the body
- Pain at night, especially lying on the affected side
- Neck movement is usually normal
- No tingling or numbness in the fingers
A simple comparison
| Feature | Cervical problem | Frozen shoulder |
|---|---|---|
| Where it hurts most | Neck, shoulder blade, down the arm | Deep in the shoulder, upper arm |
| Pain below the elbow | Common | Uncommon |
| Tingling or numbness | Common | Rare |
| Moving the neck | Often increases pain | Usually not affected |
| Moving the arm | May be fine | Restricted and painful |
| Someone else moves your arm | Usually moves normally | Stiff, limited |
Can you have both?
Yes. Many people over 40 have some wear in the neck and may also develop a frozen shoulder. A doctor can examine both areas and decide which one is causing most of the symptoms.
How doctors tell them apart
- History: where the pain started, whether there is tingling, what movements make it worse.
- Examination: testing neck movement, shoulder movement, reflexes, strength and sensation in the arm.
- Imaging: an X-ray or MRI of the neck if a nerve problem is suspected; X-ray or ultrasound of the shoulder if needed.
What helps each condition
For cervical problems:
- Posture correction, especially with phone and laptop use
- Neck and upper back exercises from a physiotherapist
- A supportive pillow
- Medicines prescribed by a doctor when needed
For frozen shoulder:
- Gentle, then progressive shoulder stretches
- Warm compresses before exercises
- Pain control and sometimes injections, as advised by a doctor
For both:
- Warm oil massage and fomentation can help muscles relax.
- Regular breaks from screens.
- Good sleep and stress management.
A note on Ayurvedic products for neck pain
Several Ayurvedic formulations for neck and spine stiffness contain bhasmas and mineral or metal-based ingredients. Some of these, such as preparations containing Ras Sindur, are mercury-based. I would advise readers not to take such products unless they come with heavy-metal testing reports and are prescribed by a qualified practitioner who knows your full health history. Herbal options such as Shallaki, along with oil massage and exercise, are gentler places to start, after discussing them with your doctor.
Red flags: see a doctor urgently
- Weakness in the arm or hand that is getting worse
- Numbness that spreads, or loss of fine hand control
- Problems with balance or walking
- Loss of bladder or bowel control
- Neck pain after a fall or accident
- Fever with neck stiffness
- Left shoulder, arm, neck or jaw pain with chest pain, sweating or breathlessness. This may be a heart emergency. Call 108 or go to hospital immediately.

Frequently asked questions
Can cervical spondylosis cause frozen shoulder?
They are different conditions, but neck problems can reduce how much you use your shoulder, which may contribute to stiffness.
Should I wear a cervical collar for shoulder pain?
Not unless a doctor recommends it. Collars are not a treatment for frozen shoulder.
Does long phone use cause neck or shoulder pain?
It can contribute to both. Holding the head forward and the shoulders hunched for hours strains the neck and upper back muscles. Raising the phone to eye level and taking regular breaks helps.
Which doctor should I see?
An orthopaedic doctor is a good first step for both. A neurologist may be involved if nerve symptoms are significant.
The bottom line
Pain that travels down the arm with tingling usually points to the neck. Stiffness in all directions with a deep shoulder ache points to frozen shoulder. Many people have a bit of both. A proper examination is the fastest route to the right treatment.
